Come join the Divine Guardian LLC team as a full-time Hospice Social Worker, where you will play a vital role in providing compassionate support to patients and their families during challenging times. Responsibilities:
- Conduct comprehensive assessments to identify patient and family needs.
- Provide emotional support and counseling to patients and their families.
- Develop and implement individualized care plans in collaboration with interdisciplinary teams.
- Facilitate communication between patients, families, and healthcare providers.
- Assist with accessing community resources and support services.
- Document patient interactions and care plans in compliance with regulations.
- Participate in team meetings and contribute to quality improvement initiatives.
- Maintain confidentiality and adhere to ethical standards in all interactions.
Requirements:
- Master's degree in social work (MSW) from an accredited program.
- Current state licensure as a Licensed Clinical Social Worker (LCSW).
- Minimum of 2 years of experience in hospice or palliative care settings.
- Strong interpersonal and communication skills with a compassionate demeanor.
- Ability to work collaboratively in a multidisciplinary team environment.
- Proficient in case management and crisis intervention techniques.
- Knowledge of community resources and support services for patients.
- Flexibility to manage a varied caseload and adapt to changing needs.
